IRB Project ID#: 20491 Title: Dog owner survey You must be 19 years old or older to participate in this database. If you are 19 or older, please take a moment to complete our questionnaire to enter your dog in our database. The questionnaire should take 10 minutes, and it's important that you fill it out as completely as possible. The information you provide will be used to select you for studies but may also be used in the studies and published publicly. Any information that could identify you will be kept strictly confidential in a secure database. Personal information will not be shared with anyone outside of the study team. You can withdraw your dog from the experiments at anytime by emailing the lab at cchil.unl@unl.edu. The submission of this form does not obligate you to participate in any way, nor does it guarantee your dog’s placement in our database. There are no direct risks or benefits to you as a result of being in this research study. We will not pay you to take part in this study or pay for any out of pocket expenses related to your participation.
